In this course, Lesa Schlatman, RN, BSN, examines processes that can be implemented for patients who are at home and for patients who have been discharged from an inpatient setting and are returning home. Learn how to combine all of the key elements of the med rec process, across the continuum of care, into one collaborative process. Plus, explore ways that pharmacies can promote patient success through ongoing involvement, and review all the pharmacy processes that have been shared.
