GENERAL DESCRIPTION

Promotores or Promotoras de Salud (community health workers) are responsible for providing health education and family resource and support services to families/individuals in the NHHC catchment area. These employees work on specific health-related projects and also assist with general educational efforts undertaken by the New Horizon Health Center.

Promotores or Promotoras de Salud (community health workers) function as a member of a collaborative health care team to create and maintain a Patient Centered Medical Home where emphasis is on excellent communication, support of the patient is provided and tracked in a culturally competent, behaviorally enhanced setting supported by information technology.

Due to the nature of NHHC services, it may become necessary to require employees to work extended hours or other variations of the usual shift to ensure adequate care of patients and to maintain services to the community.

CRITICAL JOB ELEMENTS

I. Must greet customers, patients and general public in person or over the telephone in a courteous manner.

II. Actively participate in training provided by NHHC and collaborators.

III. Talk with colonia/community residents about their concerns including those related to health, social services, political empowerment and others.

IV. Plan and lead educational sessions, discussions and walking groups with colonia/community residents.

V. Make referrals to family support, health and social service agencies in the NHHC catchment area and outside the area as needed.

VI. Follow-up on all referrals and help people access the services they need.

VII. Document all activities on forms provided by NHHC and its funding partners.

VIII. Represent NHHC professionally in meetings with health care providers, colonia residents and others.

IX. Participate in coalition meetings.

X. Conduct individual encounter and home visits as required by individual programs.

XI. Must maintain confidentiality at all times according to HIPAA laws and NHHC policy.

XII. Other duties as required.

XIII. Employee must abide by all policies including Employee Handbook, Safety Policies, Code of Conduct, and PCMH Guidelines.

XIV. Must abide by dress code policy in order to maintain a professional and neat appearance. (For details see Employee Handbook dress code policy).

XV. Employee must participate in Performance Improvement Program.

XVI. Employee will understand and have knowledge of PCMH.

XVII. Employee will serve as a member of the PCMH Primary Expanded Care Team.

JOB SPECIFICATIONS

Education Experience and Requirements:

· High School Diploma or GED. College hours preferred. Pursuing a degree in communications, public health or related field. Texas Department of State Health Service Promotor/a certificate preferred.

· Valid TX driver’s license and own transportation.

· Experience performing and coordinating outreach activities.

· Experience in a medical/service oriented facility.

· Present educational material and train in groups or one on one.

· Work flexible schedules including evenings and weekends.

· Some overnight travel may be required.

Knowledge, Skills and Abilities:

· Leadership abilities in order to motivate and promote change and coordinate activities/projects.

· Bilingual in Spanish and English strongly desired to communicate verbally and in writing, English and Spanish.

· Ability to work with the public from different walks of life and be sensitive to the cultural needs.

· Ability to operate PC, keyboarding skills and knowledge in Microsoft programs

· Ability to conform to established policies and procedures

· Ability to meet the demands, fast pace and with constant interruptions

· Knowledge of THHS programs preferred

· Effective presentation skills

· Organizational and multi task skills and attention to detail.
